Physician (Primary Care) - Recruitment/Relocation Incentive

(this is an open continuous announcement) The Primary Care Physician (PACT Team) is accountable for adhering to established policies and standards of care/standards of practice in evaluating, tracking, and following up on patient care and works in collaboration with heath care professionals to provide longitudinal case management of the PCP's entire PACT panel with independent care delivery for acute/same day needs, or when otherwise requested by the PCP. Duties and responsibilities include but are not limited to: Provide independent care delivery for chronic, acute/same day medical needs.
