Registered Nurse- Discharge Coordinator

The Nurse Admission/Discharge Coordinator Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex patient care. The nurse is an active member of an interdisciplinary team whose primary focus is on case management and care transitions to and from an inpatient setting. Job duties include, but are not limited to: Assist with patient planning assessments, identification of barriers, and activities required to successfully transition the patient in and out of the acute inpatient setting Apply the nursing process which includes systematic data gathering, assessment, and appropriate nursing judgment to ensure positive patient outcomes Coordinate and collaborate with other VA and non-VA departments, community agencies, and healthcare providers to ensure patient needs are addressed Collaborate in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines Demonstra
