Social Worker (Program Coordinator) - Care Coordination and Integrated Case Management (CC&ICM)

Major duties include but are not limited to the following: The incumbent must function with a high degree of independence and knowledge of VHA regulations in managing medically and psycho-socially complex cases. The incumbent is responsible for advance level case management and coordination of care for patients across the continuum of care, including initial assessments, recommendations, VA & community referrals, Hospice/Palliative care, supportive counseling, grief/bereavement for patient's families/friends, care coordination, and discharge planning. Demonstrate a high level knowledge of veterans' benefits and services relating to, but not limited to, special programs, VA funeral & burial services, service-connected compensation and non-service connected pension. She/he will collaborate and facilitate referrals based upon veterans' needs and eligibility. Independently conduct psychosocial assessments.
