Promotes High Reliability Organization practices (e.g., huddles, briefs/debriefs, safety rounds, closed-loop communication); provides and coordinates staff training to strengthen a culture of safety. Reviews and manages all patient safety reports; coordinates investigations, assesses risk and harm, ensures timely closure, and maintains data integrity. Analyzes data to identify trends and prioritize risks; communicates findings to leadership and ensures resource alignment for mitigation efforts. Ensures compliance with reporting requirements; coordinates sentinel event investigations and Root Cause Analyses (RCA) per age
