Social Worker (Mental Health Residential Rehabilitation Treatment Program) - EDRP Authorized

Total Rewards of a Allied Health Professional Duties include, but aren't limited to: Provide direct services to Veterans residing in the residential rehabilitation program, including those in substance use and homeless domiciliary programs. Coordinate care activities such as groups, case management, passes, patient education, interdisciplinary activities, treatment planning, and discharge planning. Complete psychosocial screening and assessment in collaboration with the interdisciplinary team, Veterans, and family members or significant others. Identify Veterans' strengths, limitations, internal and external supports, service needs, functional status, and safety needs. Provide referrals, consults, and education related to VA programs, community resources, entitlements, and services. Advocate for Veterans and families when in their best interest, while encouraging Veterans to advocate for themselves.
