Clinical Pharmacy Specialist - Inpatient

The incumbent will carry a scope of practice and work directly with the acute care medical teams performing medication reconciliations, monitoring labs and pharmacokinetics, making therapeutic adjustments, providing discharge counseling, while also serving as a primary preceptor for our robust pharmacy education program, working with our interns and residents. Major duties include but not limited to: Provide direct patient care and will be qualified and authorized to provide services in inpatient clinical wards/units, Emergency department, long-term care and virtual care settings as outlined in the CPS scope of practice. Provide appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers as appropriate.
