Social Worker - Behavioral Health Interdisciplinary Program (BHIP)

The Social Worker duties include, but are not limited to: Performs case finding functions and outreach, identifying and screening patients for psychosocial needs. Conducts psychosocial assessments and develops treatment plans in collaboration with the veteran/family and with the BHIP treatment team. Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs. Assess and identifies high-risk veterans and follow protocol for suicide prevention. Utilizes principles of human growth and development over the life span and will be able to assist Veterans in coping with the loss and grief experiences from disability and terminal illness. Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members.
