Clinical Pharmacist

Handles routine medication-related activities. Makes recommendations to clinical staff regarding medication therapy. Documents recommendations and interventions. Accurately fills or checks medication dispensing units, prescriptions, and aseptically prepared intravenous admixtures and other aseptically prepared pharmaceuticals. Fills or causes to be filled all physician orders and requests (inpatient and outpatient) in a timely manner. Track and find resolution in the Joint Patient Safety Reporting system (JPSR) pertaining to pharmacy service in a timely manner. Track and find resolution in Patient Advocate Tracking System (PATS) pertaining to pharmacy service in a timely manner. Documents all patient medication teaching sessions and consults in the patient electronic record. Provides drug information and education to medical staff, nursing staff, pharmacy staff, other health care personnel, patients and their representatives on medication-related topics as needed or assigned.
