Registered Nurse - Inpatient Case Manager

The RN ICM is supervised by the Nurse Manager for Utilization Management and Inpatient Case Management for Chief of Staff Office. The Inpatient Case Manager (CM) Nurse is responsible for the following duties: Assesses, plans, implements, coordinates, monitors and evaluates client (i.e. patient, family and other disciplines) care activities, while ensuring and facilitating the achievement of quality in a cost-effective manner. Current knowledge of trends and issues in health care. Communicates both orally and in writing. Utilizes critical thinking/sound clinical judgment in decision making. Practices competency in a variety of computer programs. Maintains knowledge of The Joint Commission standards and other external regulatory requirements. Develops and coordinates seamless transitioning of patients throughout the inpatient care process with a focus on discharge planning needs. Self-directed, flexible, and adaptable in a variety of situations. Communicates and effectively collaborates
