Social Worker (Program Coordinator) Care Coordination and Integrated Case Management

Collaborate with internal stakeholders to assess CM composition, structure, and services to identify promising practices and gaps in CM services and identifying opportunities to enhance communication, collaboration, and coordination across the care continuum. Guide medical center through the CCICM sustainment and develop/review annual CCICM Education Plan. Coordinate and collaborate with the facility level care coordination, care management, and case management teams to include but not limited to nurse and social worker navigators, care coordinators, care managers, and case managers.
