Social Worker (Primary Care)

This Social Worker position is in Social Work Service at the VA Healthcare Systems, located at the Jamaica Plain campus. This position is a full-time at 40 hours per week. The Primary Care Social Worker provides independent clinical social work services to eligible Veterans with complex medical, mental health, and psychosocial needs. Clinical services include assessment, treatment planning, case management, crisis intervention, brief supportive/counseling, advocacy, consultation, and coordination of care with VA and community resources. Major duties include, but are not limited to: Conduct comprehensive psychosocial assessments and develop to address psychosocial concerns. Provide individual and clinical case management. Coordinate services with interdisciplinary treatment teams, VA programs, and community agencies to ensure continuity of care.
