About the role
The NP has prescriptive authority for a panel of patients or program as indicated by their position or assigned by their supervisor. The major duties include, but are not limited to: Independently manages or in collaboration with attending physician(s), a panel of patients with acute and chronic health conditions with emphasis on health promotion and disease prevention using evidence-based practice. Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with the interdisciplinary team. Initiates referrals and consults to specialty services. Provide End-Of-Life care and symptom management to all patients in an inpatient and outpatient setting. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
