COMMUNITY CARE NETWORK (CCN) CARE MANAGER REGISTERED NURSE

Duties and responsibilities of the COMMUNITY CARE NETWORK (CCN) CARE MANAGER REGISTERED NURSE include, but are not limited to: Community Care Network (CCN) Care Manager, is a registered nurse (RN), who executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care within VHA and VA-Community Care program. RN has the primary responsibility for providing clinical expertise in coordinating Veteran care under the care for Short Term Rehab (30, 60, 90 and up to 100 calendar days) in a Skilled Nursing Facility, in the community for planned and unplanned/emergent episodes of care, and discharge disposition needs. This includes receipt of initial requests for transfers from community hospitals to skilled nursing facilities under OPTUM. The RN provides close monitoring of continued stay reviews, maintaining nursing standards across service lines, as well as being able to multitask, be flexible and adaptable in the evolvin
