Registered Nurse-Home and Community Based Care-Outpatient Staff

As the Home and Community-Based Care (H&CBC) Registered Nurse assigned to the Veteran Directed Care (VDC) program is responsible for providing care coordination and program monitoring for Veterans with high levels of functional, medical, and psychosocial needs. This role ensures Veterans receive safe, high-quality, and cost-effective care while maintaining autonomy and remaining in their preferred care setting. Duties of the VDC RN include, but are not limited to: Assessing and determining clinical appropriateness of Veterans for enrollment in the Veteran Directed Care program. Implementing and applying the Case Mix Index (CMI) to establish appropriate levels of care and budget allocations. Developing, reviewing, and monitoring individualized Veteran budgets (spending plans). Monitoring the quality and safety of care provided to Veterans enrolled in VDC.
