About the role
The Mental Health Care Facilitator perform independent, comprehensive telephone and clinical triaging in the primary care setting, supporting primary care providers in their treatment of patients with mental health conditions. Monitor patient progress and symptom severity by conducting a structured telephonic interview and using validated tools. Assist patients with treatment plan adherence. Coordinate patient referrals to support services provided through the internal and external community, or the TRICARE network. Responds to emergency situations. Assist clinic with patient and staff education and provide individual and small group training in order to increase referrals and promote understanding of the role of the mental health care facilitator. Follow quality improvement and internal control activities for the ambulatory care setting to include risk appraisals, conducting chart reviews, identifying trends and reporting corrective actions.
