Practical Nurse (Outpatient)

Identify and maintain medical histories on patients. Communicate with patients to ensure they are informed about care plans. Prepare and deliver oral, topical and injectable forms while monitoring for side effects/adverse reactions. Document accurate administered medications and patient responses promptly in compliance with facility policies. Assist in performing medical procedures, physical examinations and treatments. Collaborate with healthcare team to develop and adjust care plans based on patient progress and feedback.
