Social Worker - HUDVASH

Position vacancies are at Gainesville VAMC and Jacksonville locations. This position is located at the North Florida/ South Georgia Veterans Health System in the Community Reintegration and Recovery Service. The incumbent provides community-based case management services to Veterans participating in the HUD VASH program. Works closely with the VA medical center's homeless programs, residential and community programs, and domiciliary programs to ensure that coordination between these entities is facilitated and Veterans direct care, referral, and follow-up is based on a continuum of services. Independently provides clinical psychosocial and case management services for eligible Veterans and their family members in the program. Works in close consultation with the HUDVASH Social Worker assigned. Veterans eligible for HUDVASH come from a wide geographic area. Their health and mental health needs may be complex and require a high degree of clinical oversight and creative problem solving. S
