PRACTICAL NURSE (FAMILY MEDICINE)

Obtain information from patient, family, and guardians to identify reason for seeking health care. Perform behavioral health screening and take patient vital signs. Document patients' chief complaints and other pertinent information in the health record. Facilitate care coordination utilizing nursing assessment to communicate relevant information according to the medical plan of care. Prepare assigned rooms for scheduled patients. Assist the physician with various examinations and diagnostic procedures. Educate patients, family regarding self-care procedures and post-care reactions.
