SOCIAL SERVICES ASSISTANT

Conducts comprehensive screening interviews with Family Advocacy Program clients. Explains appropriate FAP services, requirements, policies, and procedures while obtaining pertinent biopsychosocial history and other case information to be incorporated into client files at the assigned installation Assists in the development and implementation of comprehensive outreach and prevention programs for individuals, families, and groups requiring assistance with family-related issues. Contributes to action plans for Family Advocacy's Secondary Prevention and Client Engagement team based on community needs assessments and program objectives specific to the installation's demographic profile and identified risk factors. Coordinates resource and referral services between military and civilian community agencies to ensure comprehensive support for military families.
