Social Worker (Mental Health Consultant)

Apply biopsychosocial evaluations to propose mental health interventions. Provide evidence-based treatment, including behavioral, cognitive, and motivational approaches. Provide primary care providers and staff regular and concise feedback on the status of referred patients, to include recommendations for team-based care. Implement psychoeducational classes to target high frequency diagnoses found in the primary care clinic. Document information in the electronic health record during patient encounters.
