Senior Social Worker - Primary Aligned Care Team (PACT)

Total Rewards of a Allied Health Professional The PACT Social Worker Lead provides leadership, direction, orientation, coaching, in-service training, staff development and continuing education program for primary care social work staff and assist in planning, establishing and implementing policies and procedures; developing individual or group goals and objectives; and monitoring, coordinating and overseeing the clinical practice of social work staff in addition to providing direct Veteran care as a primary social worker. Duties may include but are not limited to: Establishes and maintains effective therapeutic relationships with Veterans and/or their families in the Primary Care Clinic. Works independently with Veterans and/or their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, financial, legal, and psychosocial problems.
