Social Worker (Trauma)

Provides professional Social Work interventions to patients and family members affected by traumatic incidents. Provides proactive intervention in discharge planning activities for trauma patients. Consults with other staff, civilian, military resources and community agencies to plan and coordinate patient care throughout the continuum. Participates in development, implementation, and documentation for Quality Assurance. Provides outpatient case management services for designated patient population. Serves as consultant for Civilian Care Coordination. Serves as trauma consultant to staff and providers on psychosocial issues, family dynamics and adjustment to illness. Screens traumatically injured patients for potential drug and alcohol use/abuse.
