VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws Duties may include but are not limited to: Designs, implements, assesses, and monitors therapeutic drug plans to achieve positive patient outcomes through direct and indirect interactions with patients, providers, and staff in assigned areas. Therapeutic medication plans including but not limited to assessments of clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions and intolerances, cost-effectiveness, formulary considerations, antimicrobial stewardship, nutrition support, and therapeutic outcomes, as required. Findings are acted upon based on scope of practice and/or communicated with prescribers and provides appropriate alternatives to current treatment plans, as needed. Demonstrates expertise in the principles of clinical pharmacokinetics and pharmacodynamics and is knowledgeable and able to make dose adjustment recommendations based on objective laboratory findings.
