Program Specialist - Patient Centered Management Module (PCMM)

Some of the duties include, but are not limited to: Coordinate and manage patient care team setups, defining roles, and assigning patients and staff to teams. Ensure accuracy and integrity of the provider panel database across the facility and affiliated clinics. Serve as a resource for clinical and administrative staff, providing guidance and troubleshooting support for software applications (Vista, CPRS). Prepare and complete reports, analyze data discrepancies, and ensure data integrity, resolving transmission errors. Provide assistance and technical support for management and operating program analyses. Compile and present statistical reports to various stakeholders, ensuring compliance with guidelines and directives.
