Nurse (Clinical/Case Management)

Maximizes and coordinates healthcare services and resources available at Military Treatment Facility. Conducts comprehensive clinical interviews with the patient and other family members. Recognizes the need for improvement and offers solutions to a wide variety of management processes. Consults and coordinates with other multi-disciplinary care professionals to assist patients to optimize their level of function and self-care. Serves as liaison between the patient and the health care professionals. Coordinates services, referrals, and appointments for each patient under case management services. Assess patients with various complex medical conditions daily.
