Additionally, identifies health related conditions while the patient is being evaluated. Including, but not limited to: new patient medication intake reviews and polypharmacy screening. Routinely reviews the computer medication profile for prescription duplication, therapeutic duplication, prescription noncompliance, and allergies/adverse drug reactions. Reports identified problem patients to the assigned clinical pharmacy specialist for evaluation. Routinely reviews laboratory monitoring reports for selected disease states and identifies patients with values outside of the specified parameters. Reports identified patients to the assigned clinical pharmacy specialist for evaluation. Inputs non-controlled new and renewal prescriptions into the computer system on behalf of the provider (subject to provider's authorization signature). Prescriptions entered by the clinic
