Nurse (Clinical/Case Management)

Serves as Nurse Case Manager responsible for analyzing problems associated with providing health care to patients and/or beneficiaries for the Military Treatment Facility (MTF). Conducts comprehensive assessments of beneficiary’s health needs to develop a plan of care. Educates both patient and members of the healthcare delivery team about case management, healthcare and treatment options, community resources, insurance benefits, psychosocial concerns, etc. so that informed decisions can be made. Consults and coordinates with other multi-disciplinary care professionals, agencies, organizations and other ancillary support systems. Serves as a liaison between the patent and the health care professionals.
