RN - Clinical Transfer Coordinator

The Clinical Transfer Coordinator Registered Nurse is responsible and accountable for the coordination, facilitation, and oversight of patient transfers and referrals within the Veterans Health Administration (VHA) and between VHA and community partners. The incumbent executes all elements of the nursing process-assessment, planning, implementation, and evaluation-while ensuring safe, timely, and cost-effective transitions of care across the continuum. Major Duties and Responsibilities: Coordinates all interfacility transfers, to include VA-to-VA and VA-to-community transfers, ensuring safe, efficient, and patient-centered transitions of care. Serves as the central point of contact for all incoming and outgoing patient transfer requests. Facilitates communication between referring and accepting providers (physician-to-physician and nurse-to-nurse) to ensure continuity of care.
