Social Worker (Program Coordinator)

Provide supportive services and case management to Veterans residing in Contract Emergency Residential Housing facilities. Utilizes various treatment modalities of individual and group work, casework, community organization and community outreach activities to alleviate or remove problems that interfere with the Veteran's ability to achieve independence. Monitors the clinical care and asses the compliance of the programs receiving payments, as described in the recipient's contract. When necessary, the incumbent intervenes to facilitate compliance or corrective actions and, when appropriate, develops, facilitates and monitors plans of correction. Provides oversight of the Contract Residential Services as defined by VHA policy. Serves as the Contracting Officer Representative (COR) for the Contract Emergency Residential Housing Program, upon completion of required training and certification. Coordinates care and acts as an advocate on behalf of Veterans to ensure comprehensive service de
