Advanced Practice Nurse (Nurse Practioner)

Conducts and directs collaborative care assessment, diagnosis and management of patients in both the inpatient and outpatient settings. Assesses acute and non-acute clinical problems. Consults and collaborates with physicians and other health care providers regarding patient management o include evaluating and revising therapeutic plans and managing high-risk, unstable, or extremely complex patients. Evaluates care delivery models and strategies, using concepts related to community, environmental/occupational health, and cultural and socioeconomic dimensions of health. Educates patients and their families regarding healthcare processes and diagnostic tests; therapeutic regimen, including medication, diet, follow-up care; and methods of adapting medication and treatment to current lifestyles.
