Registered Nurse-Patient Aligned Care Team (PACT)

The Outpatient Staff Registered Nurse (RN) delivers direct, gender-specific care and coordinates services for Veterans within the Primary Care Department. This role is responsible and accountable for independent nursing practice and for making appropriate decisions about the delegation of tasks to ensure safe, high-quality care. Provides essential, evidence-based nursing care to assigned patients while building technical and clinical competencies. Applies the nursing process (assessment, diagnosis, planning, implementation, evaluation) under close supervision. Delivers Veteran-centered care with attention to patient experience, safety, and satisfaction. Communicates and collaborates effectively with the interprofessional team, demonstrating growth toward independent team leadership. Utilizes educational resources and self-evaluation to support ongoing professional development. Incorporates evidence-based practice and patient preferences into care to improve outcomes and resource use. V
