Social Worker (Clinical)

Work with highly complex cases in which individuals are severely impacted by moderate to severe psychiatric diagnoses. Provide individual and group therapy using various modalities as clinically indicated to include brief problem-focused psychotherapy, cognitive-behavioral therapy, and client-centered therapy. Collect bio-psycho-social information about patients using interview, case history, observation techniques, and assessment methods and reviews notes and information collected to identify problems and concerns. Develop individualized treatment plans based on psychiatric diagnosis, identified problems, and treatment needs for each case. Provide on-going consultation services for a sub-set of patients who require on-going monitoring and follow-up (i.e., continuity consultations) with providers where appropriate. Evaluate treatment effectiveness using objectives and measurable outcomes to make adjustments if appropriate for increased compliance and efficacy. Work closely with multi-d
