Mental Health Case Manager

Serve as expert mental health case manager and member of multidisciplinary team of medical and professionals to identify high risk and high-cost diagnosis. Develop, implement, communicate and coordinate individualized empirically informed patient treatment plans that address all diagnosed conditions and health concerns. Communicate with treatment facilities, both in direct and purchased care to coordinate patient care and transitions. -Communicate/coordinate with appropriate parties including command and other medical teams within the guidance provided by DHA. Track client progress towards goals, modify care plans as needed, and advocate for clients' rights and preferences.
